Good news, Warriors!  Warrior Massage is now accepting/billing Labor & Industries and auto accident claims for massage!  If you have an open auto accident injury claim OR work injury claim, please contact me (email to [email protected] works best) to find out what you'll need to do to get started.  In general, you will need:

• Claim information including:
  
  • claim number
    
  • date of injury
    
  • phone number & address of insurer/claims dept.
    
• Prescription from the attending physician (the primary doctor/provider managing care for your injury... this could be your primary care doctor, or it could be a doctor, chiropractor, or other specialist you are seeing just for this injury).  Prescriptions should include: 
  
  • AP's (Attending Physician's) name, address, phone, and NPI (National Provider Identifier number)
    
  • AP's Diagnosis, including the diagnosis code
    
  • AP's prescription for number and frequency of massage treatments
