Cancellation/No-show
In order to cancel or reschedule an appointment, you must give at least a 24-hour notice or you will be charged a cancellation fee for the time that was reserved for you. If the spot you reserved is filled in time, the cancellation fee will be waived. If you have not arrived or called within 15 minutes after the start of your appointment start time, it will be considered a no-show and your appointment will be cancelled. The cancellation fee is 50% of the cost of the session.
If you have a fever (>100F) and/or or other symptoms of COVID-19 OR influenza, or have recently come into contact with an infected person and believe you may be infected, please call to reschedule your appointment as soon as you become aware of it. If you notify us of your illness and reschedule before your appointment time, cancellation fees will be waived.

Gratuities (Tips)
Your gratitude and appreciation for the service you receive at Warrior Massage is graciously accepted, however cash gratuities are not. In lieu of tipping, please consider referring others to Warrior Massage, writing positive reviews on your favorite review site (ie Yelp, Google+, Facebook, etc), or simply booking another appointment at Warrior Massage. Cash left in the treatment room will be returned; if it is not found until after you depart, it will be applied as a credit on your account towards your next massage appointment.
Payment
Payment is collected before service begins. Any coupons, vouchers, or other special deals must be presented with payment or will not be accepted. Accepted forms of payment include: cash, check, and credit card. Checks should be made payable to Warrior Massage. If you are paying cash, please bring exact amounts; we do not keep a cash till and no change is kept on-site at Warrior Massage. If you bring cash and are not able to bring exact change you may elect to have the difference credited to your account for future services.
Patient Privacy
Warrior Massage collects certain information from every patient, including demographic information such as address, phone number, etc, and medical information such as health history, medications, current conditions, etc. This information is used by Warrior Massage solely for providing the best service to patients and ensuring compliance with Department of Health standards of care. Information is kept secure in a manner compliant with the HIPAA (Health Information Portability and Accountability Act) and HITECH (Health Information Technology for Economic and Clinical Health) Acts.
Under no circumstances will any information collected by Warrior Massage be shared with any third parties, except with the express written permission of the patient (or the patient's legal guardian in the case of minors). For example, permission may be sought to discuss your case with your primary care physician if it is necessary to properly coordinate care, or to transmit records to attorneys involved in litigation involving your healthcare.
Patients' demographic information will never be sold or exchanged with any third party marketing groups or entities. Information may be used for internal marketing purposes with the express written permission of the patient, which a patient may opt in or out of at any time.

Safe Space
Warrior Massage is a safe space for all people. Manual therapy is for everybody with a human body, and it is important to us that all patients and guests feel safe and welcomed.
